It is not a mandatory document but sometimes it may be attached with other important documents as part of the NEET application. NEET domicile criteria includes domicile based reservations. These reservations based on residence have been eliminated from NEET PG. But for NEET UG, students may take admissions into MBBS, BDS, BUMS, etc courses through the NEET state quota. 

NEET UG reserves 15% seats for the All India Quota and 85% seats for state quota. Candidates living for more than 3 years in a particular state can apply for the 85% NEET state quota seats.
